Patient Information and FAQ

Register as a Codman patient in four easy steps:

  1. Contact our Call Center at (617) 822-8200 to speak with a representative. Alternatively, new patients may walk into the Health Center at 637 Washington St., Dorchester, to schedule an appointment in person.
  2. Tell the Call Center representative that you’d like to schedule an appointment at the Mobile Health Clinic at a service site convenient to you.
  3. Our Call Center staff will work with you to assign you a primary care provider, assess your insurance status, and schedule your appointment.
  4. If you need assistance enrolling in health insurance, our Call Center can schedule an appointment for you with our Insurance Enrollment Team, who will help you find a plan that works for you and your family.

What to bring to an appointment with Insurance Enrollment:

For patients applying for MassHealth:

  • Photo ID, if available
  • Social Security numbers for household members applying, if available
  • Proof of Massachusetts address, such as a lease or recent utility bill
  • Proof of income, such as recent pay stubs, tax returns, unemployment benefits, or Social Security benefit statements
  • Any MassHealth letters or notices received

For new patients:

  • Insurance card
  • Photo ID, if available
  • Medical records, if available

Walk-ins are welcome and will be accommodated based on availability.

Community members who are not currently Codman Square Health Center patients, and do not have an appointment at the Mobile Health Clinic can “walk-in” to the clinic and request to be seen. Codman staff can register you onsite and schedule an appointment.

“Walk-in” to request an appointment:

If you see the Mobile Health Clinic in your neighborhood, and you would like to be seen by medical staff, speak to our Enrollment Representatives at the mobile clinic host site and request an appointment.

Our staff will:

  1. Determine if there are open slots on the schedule for a same-day appointment. If there is same-day availability, our staff will begin the patient registration process and schedule an appointment for you. If there is not same-day availability, our staff will assist you with scheduling a visit at a time and location that works for you.
  2. Register you as a patient of Codman Square Health Center. We can begin the patient registration process onsite at the mobile clinic. Our team will work with you to assess your insurance status and assign you a provider. If you are unable to complete the registration process at the mobile clinic, our staff will connect you with our Insurance Enrollment team to assist you in registering.

The first time you visit any Codman location, you should:

  • Know any health conditions you have currently or have had in the past
  • Know any major health conditions that your family (parents, siblings, grandparents) has currently or has had in the past
  • Bring your health insurance card if you have one (you will need this card at every visit)
  • Bring a list of any medications you are taking (or bring the medications themselves)

During your first visit we will:

  • Help you fill out necessary forms
  • Help you choose a primary care provider
  • Explain how to make appointments and get access to all of the Health Center’s services
  • Make any appointments you need with providers and/or with insurance services

By the end of your visit, you should know:

  • The answers to all of your questions; if not, schedule a follow-up visit, or check with registration to determine who may best answer your questions
  • What your treatment plan is (i.e., medications, referrals, other services, follow-up visits)
  • What medications you will be taking, how to take them, possible side-effects, and why you are taking them
  • The date and time of any lab tests or follow-up appointment

If you are ever unable to attend a scheduled appointment:

  • PLEASE CALL 24 hours in advance if you cannot keep your scheduled appointment
  • Your cancellation will allow us to offer that appointment time to another patient
  • Please keep your records updated; tell us about changes in your address, telephone number, or other contact information.

Our navigators work with MassHealth and the Health Connector to get those who qualify enrolled in a plan that works for your health and financial needs.

STEP 1: What you need to bring

For patients applying for MassHealth:

STEP 2: Do you qualify?

Patients may qualify for MassHealth, Careplus, MassHealth standard, or MassHealth Family Assistant, or Wellsense Community Alliance.

STEP 3: Schedule your appointment

After enrollment, Insurance Enrollment staff will assist the patient in scheduling an appointment with a Codman Square Health Center provider.

Are you over 65?

To complete The MassHealth Over 65 application, we will support the patient with required documents. Please bring:

  • Proof of income
  • Bank statement
  • Pension documentation (if applicable)
  • Social Security income
  • Life insurance documentation
  • Copies of any deeds
  • Statement of investments

With all the documents, we will complete the application with MassHealth. This process can take up six weeks.

No other insurances? Sliding Scale Fee Discount

If the patient has no active insurance, financial counselors on our Enrollment team will assist the patient applying for MassHealth. Based on the patient’s yearly income, staff will use documentation to determine the sliding fee discount.

At your appointment, please bring proof of identity, proof of residency, and proof of all sources of income. Based on your income, you will be enrolled in a health plan that you can afford and that works for you. Lack of insurance should not prevent you from asking for help or learning about available services.

A: Yes. Codman Square Health Center provides interpreter services at no cost to patients with limited English proficiency and provides communication assistance for patients who are hearing impaired.

Q: Is the Mobile Health Clinic accessible to people with disabilities?
